How do I enroll?

The process to enroll is simple. Proxy access allows a parent or guardian to create a Children's Connect account, then access information about each minor child or dependent. The entire process can be done in three easy steps.

Please note: if you received your activation letter containing your activation code during your last office visit, skip to Step 3 in the instructions below.

1. Fill out our Consent Form. You may either fill out the form the next time you visit your Children's Physicians office or you can fill it out and home and mail it to your Children's Physicians office. To find your Children's Physicians office address, click here.

2. After the Consent Form is completed, turned in and approved, you will be issued an activation letter that contains your Children's Connect activation code. If you filled out your Consent Form in the office, you will be given your activation letter in the office during that same visit. If you mailed in your Consent Form, we will mail you your activation letter. You must have your activiation code to proceed with the enrollment process.

3. Once you have your activation letter that contains your activation code, log onto ChildrensPhysiciansOmaha.org and click the Children's Connect icon located on the home page. This will take you to our sign-in page. Once on the sign in page, click on the First Time User sign up botton located on the right side of the page. Then, simply follow the directions on the screen.
